Search for your Origin account username-or soldier name, whatever you like to call it-on this site, then click the blue CLAIM button on the profile page.

Follow the instruction on the new page to finish claiming your account, which means changing your BF4 nametag to the verification code, then you should be good to go.

Judging from this Snapshot of your stats, you could've been banned for so many reasons. 

I've done some investigation by myself and easliy found a lot of kill-trading battlereports. 870 MCS, M39 EMR, ACW-R, SAIGA 12K, G18, AK 5C, FN57, M4, A-91, AS VAL, P226, RFB, SVD-12 etc. This list goes on and on and on. Probably half of your kills are achieved by kill-trading. That's when I asked myself why did you buy this game from the beginning if you just want to get those "FANCY STATS" and just couldn't play this game normally like everyone else, especially when I discovered that you were trying to cover the fact of kill-trading, BY NOT AIMING FOR THE HEAD KILL-TRADING. 

So here's a quick reminder that kill-trading is clearly against EA's ToS and it can lead to a stat reset by Fairfight. If that happened, your ban will definitely not lifted.

Well, so let's get back to the topic. We currently don't ban player for kill-trading (but we might flag your account in the future), so if you claim that all of the suspicious stats on the gun I mentioned earlier result from kill-trading, provide as many battlereports as you can to prove it. 

Then the admins will decide if the ban will be lifted or not.

Honestly, I didn't find anything outside kill-trading. So I personally think that these battlereports you've already provided will definitely help if you are trying to get your ban lifted, although it might not be sufficient enough in quantity to clear the questionable stats on all the weapon I mentioned ealier. What I suggest you doing now is finding the battlereports that show an abnormal HSKR, classifying them by the guns you are using in that round if possible (for the admins to review easliy) and post them below, then just wait. Also, sorry for being purposely mean before :)
