Appeal: I most certainly did not perform any sort of "player information streaming" to any "Service". I'm unaware of performing any activity that may have caused my Account(s) to having overlapping IP and timestamp information. As far as I know, I only have one account.

Why does this account have a similar name as you had?

I'm not sure I understand... The name on the account you just linked is "SeregaZinger411". Mine is St0nierThanThou and was previously DanceTonyR.

Why would you lie about your name history?

Ah! That was a couple months ago, right? My origin account got compromised from someone overseas.  I forgot that they had changed my name to something similar.  Below is a link to my google drive folder with a screenshot from the EA support case along with a copy and paste of the entire dialogue between myself and the support rep after the account had been hacked. Hope this helps. If there's another/better way to upload the information, let me know.

Unfortunately we do not remove bans from hacked accounts. Your account is your responsibility as there are plenty of ways to prevent it from getting hacked.
